When we are closed our phones are answered by a registered nurse who is able to assess your needs. The nurse will offer advice or immediate assistance if required but cannot take messages or make appointments.

In order to enrol with us please complete the following steps.

1. Check your eligibility to enrol
2. Download and complete the Enrolment and Terms and Conditions form and the ConnectMed application.
3. Once the forms have been completed please return to us.

Please Note:
  • You can hand your forms in at Reception or you may choose to post or e-mail the forms to us together with a copy of your drivers licence or passport ID page.  If you are not a New Zealand citizen you need to provide proof of your eligibility by sending us a copy of your visa (residency or work visa) as well.
  • All adult patients are to sign the Patient Credit Terms and Conditions of Trade at the time of enrolment (attached the to enrolment form)

  • Please note that every patient over the age of 16 is required to sign their own enrolment form.
  • If you require an appointment immediately, please phone 03 352 9053  and we will try to assist you, however our current enrolled patients  are given priority and at times an immediate appointment will not be possible.  You will be asked to complete an enrolment form on arrival – please arrive 10 minutes before the appointment to complete the paperwork.

An eligible individual is:


a New Zealand citizen


holds a resident visa or permanent resident visa (includes residence permits issued before December 2010)


an Australian citizen or Australian permanent resident AND able to show that he/she has been in New Zealand or intends to stay in New Zealand for at least 2 consecutive years


has a work visa and is able to show that he/she is able to be in New Zealand for at least 2 years (previous visas/ permits included


an interim visa holder who was eligible for publicly funded health services immediately before his/her interim visa started


a refugee or protected person.

Payment Terms:  All consultations are required to be paid on the day.  Patients who do not attend their appointments will be charged a fee of $20

Repeat Prescriptions



We require a minimum of three business days notice for a repeat prescription to be processed.


Prescription charges:

Faxed $23 – Payment is to be made to our bank account 06 0817 0020081 25

Prompt payment is essential as prescriptions will no longer be faxed until the payment has been received.

Collected $23 – Payment is to made at the time of collection

  • We do not process same day prescriptions via the website but if you do require a same day prescription please phone the surgery to arrange. The cost of this service is $28.
  • Repeat prescriptions are only available for stable medical conditions which have had medication previously prescribed by one of our doctors.
  • Unless previously agreed, you must have been seen for this condition in the past 6 months. Many conditions require 3 monthly visits.

Test Results

Papanui Medical Centre policy is not to contact patients routinely for normal results. You will be notified of urgent or abnormal results as soon as possible after the doctor has reviewed them. Please ensure we have your current preferred method of contact. If you have specific concerns or have not been contacted you can ring 352 9053 and speak to the results nurse who are available from 9am-12.30pm and 2pm-5pm week days.
